Kristina K. Lee, Pharm.D., BCPS, graduated from the University of Florida College of Pharmacy in 2012 and completed her PGY1 Pharmacy Residency at South Miami Hospital in 2013. Her former roles at Baptist Hospital of Miami and South Miami Hospital include Clinical Staff Pharmacist, Antimicrobial Stewardship Program Clinical Pharmacist and Performance Improvement and Quality Pharmacist. Dr. Lee became a Board-Certified Pharmacotherapy Specialist in 2015. While at South Miami, she played an integral role in the implementation of DoseEdge and KitCheck, while serving as the preceptor for the Hospital Pharmacy Practice Management resident learning experience. In 2018, Dr. Lee joined the Pharmacy Informatics department. She is involved with various system committees, precepts pharmacy residents and students in Pharmacy Informatics and Advanced Pharmacy Informatics, and was recognized as the 2022 Informatics Pharmacist of the Year.
Jennifer Munguia, Pharm.D., BCOP graduated with her Doctor of Pharmacy degree from Palm Beach Atlantic University in 2014. Following graduation, she completed her PGY1 Pharmacy Residency at Baptist Hospital of Miami in 2015. After residency, Dr. Miller worked for the University of Miami Health System as an outpatient oncology pharmacist at one of the Broward satellites. She received her Board Certification in Oncology Pharmacy in 2017 and returned to Baptist Health South Florida in early 2018 as a Pharmacy Clinical Coordinator for the Miami Cancer Institute. She then joined the Pharmacy Informatics department in 2018 and is now an Informatics Pharmacist, specializing in oncology. She was recognized as a finalist at the 2021 Chief Digital and Information Officer Awards.
Rafael Bazan, CPhT, RPhT, is a System Analyst with the Pharmacy Informatics team. Rafael has extensive experience with pharmacy systems, as a Pharmacy Technician, Pyxis Administrator and Pharmacy Buyer at Broward Health, and then an Automation Administrator at Memorial Regional Hospital and System Analyst at Memorial Specialty Pharmacy. Rafael joined the Baptist Health Pharmacy Informatics team in 2018 and has played a role in maintaining Pyxis, Retail Pharmacy Systems, SharePoint and the Pharmacy Intranet page. He has led the transition from SharePoint to ServiceNow for work items for Pharmacy Informatics and been instrumental in the implementation of the narcotic surveillance platform ControlCheck.
Marilin Bolumen, Pharm.D., earned her Bachelor of Science in Pharmacy in 1992 from the Massachusetts College of Pharmacy and then her Doctor of Pharmacy from the University of Florida in 1999. She has over 25 years of pharmacy experience, with the majority specializing in pediatrics and oncology. Her career path has taken her to Eckerd Pharmacy, Homestead Hospital, Kendall Regional Hospital, Broward General Medical Center, Joe DiMaggio Children’s Hospital and Parallon (HCA). Just prior to joining the BHSF Pharmacy Informatics team in 2018, Dr. Bolumen was a Senior Clinical Staff Pharmacist at Baptist Hospital of Miami and Baptist Children’s Hospital, where she was instrumental in the conversion to Cerner Millennium and the implementation of DoseEdge. She earned her ASHP Certificate of Achievement in Pharmacy Informatics in 2020 and serves on the Pediatric Clinical Optimization Committee.
Jonathan Choukroun, Pharm.D., CPHIMS earned a Bachelor of Science in Computer Science from Florida International University in 2011 and then a Doctor of Pharmacy degree from Nova Southeastern University in 2015. He first joined the Baptist Health South Florida system as a Clinical Staff Pharmacist at South Miami Hospital, where he was the pharmacy lead in the transition to Cerner Millennium. In his current role as an Informatics Pharmacist, Dr. Choukroun develops and implements solutions to streamline complex workflows by leveraging the capabilities of Cerner. He received a Chief Digital and Information Officer Award for his work on the implementation of an automated process for the monitoring of heparin infusions. Dr. Choukroun became a Certified Professional in Healthcare Information Management Systems in 2021. He serves on the Clinical Decision Support, Electronic Health Record, and Data Analytics Committees.
Sukaya Devot, Pharm.D., earned her Doctor of Pharmacy degree from the University of Maryland Baltimore in 2012. She was a pharmacy technician at Doctors Hospital before becoming a pharmacist and returned to Doctors Hospital upon graduation as a Clinical Staff Pharmacist. In 2017, Dr. Devot led the pharmacy conversion to Cerner Millennium at Doctor’s Hospital and the DoseEdge implementation at both Doctors Hospital and South Miami Hospital. As an Informatics Pharmacist on the BHSF Pharmacy Informatics team, she primarily focuses on the adult patient population and supporting DoseEdge, Codonics, Logistics, and IV infusion pump drug libraries. She completed her Teaching and Learning Certificate in 2021 and was recognized as a finalist at the 2021 Chief Digital and Information Officer Awards.
Liliana Estrada Iturria, RPh graduated from the Central University of Venezuela in 2001. She spent fifteen years at Aventura Hospital as a Clinical Pharmacist, Lead Clinical Pharmacist and then Informatics Pharmacist. During this time, she assisted with Aventura Hospital’s transition to computerized physician order entry, including building and providing training. Ms. Estrada Iturria assisted Nicklaus Children’s Hospital with their transition to Cerner Millennium in 2012-2013, and the University of Miami Hospital with their transition to Epic in 2017-2018. She is proficient with Meditech and certified in Epic – Willow. Ms. Estrada Iturria joined the Miami Cancer Institute in 2017 as an Informatics Pharmacist, before transitioning to the system Pharmacy Informatics team in 2018. Her work focuses on the adult patient population and supporting the ambulatory and urgent care settings. In 2021, she was recognized as a finalist at the inaugural Chief Digital and Information Officer Awards.
Jesus Fernandez, Pharm.D. graduated from Nova Southeastern University with his Doctor of Pharmacy in 2015. Dr. Fernandez has worked as a Pharmacy Technician at South Miami Pharmacy, Walgreen’s and Baptist Hospital of Miami, and as a Pharmacist for Walgreen’s and Baptist Hospital. He has extensive experience with all pharmacy applications, with a focus on Cerner Millennium and Pyxis ES. Dr. Fernandez was integral in the initial system-wide implementation of Cerner Millennium at Baptist Health South Florida. He joined the Pharmacy Informatics team in 2018 and was appointed to the Electronic Health Record and Clinical Decision Support Committees. In 2021, Dr. Fernandez was recognized as a finalist at the 2021 Chief Digital and Information Officer Awards in the categories of improved patient experience, advances in clinical capabilities/support and business system automation/simplification.
Jayesh Patel, Pharm.D., started his pharmacy career in 2004 after graduating from St. John’s University in Queens, NY. He spent two years traveling to raise awareness for and promote healthy communities in tribal villages. After working in retail pharmacy for two years, he joined Einstein Medical Center in Philadelphia, Pennsylvania as a Clinical ICU Pharmacist in 2008. In 2013, he joined Boca Raton Regional Hospital as a staff pharmacist. Since 2014, Dr. Patel has been working as the Clinical Informatics Pharmacist and liaison of clinical pharmacy to all integrated systems at Boca Raton Regional Hospital. He holds an ASHP certification in Clinical Informatics and precepts the Clinical Informatics learning experience for students and residents. Dr. Patel serves on various committees involving informatics and has been part of multiple implementation projects involving Cerner conversions and pharmacy automation.
Celines Puig, Pharm.D., BCPS earned her Bachelor of Science in Pharmacy degree from the University of Puerto Rico in 1999 and then her Doctor of Pharmacy degree from the University of Florida in 2004. She became a Board-Certified Pharmacotherapy Specialist in 2012. She has worked at Eckerd Pharmacy, Kendall Regional Medical Center and, just prior to joining the BHSF Pharmacy Informatics team in 2021, at Jackson North Medical Center as a Pharmacy Operations Manager. While at Jackson North, Dr. Puig was part of multiple implementation projects involving pharmacy automation, IV room workflow optimization, and Cerner Millennium. As part of the BHSF Pharmacy Informatics Team, she primarily focuses on the adult patient population and supporting applications such as Bluesight for Controlled Substances and DoseEdge.
Aixa Rey, Pharm.D., earned her Doctor of Pharmacy degree from the University of Florida in 1989. After graduation, she completed a Post-Doctoral Fellowship in Family Medicine at the University of Florida, Colleges of Pharmacy and Medicine in 1991. She held the Consultant Pharmacist license and became a Board-Certified Pharmacotherapy Specialist in 1992. Just prior to being named Baptist Health South Florida’s first Director of Pharmacy Informatics, she was the Pharmacy Operations Manager at West Kendall Baptist Hospital. Dr. Rey was a reviewer for the Advarra Institutional Review Board and is a current referee reviewer for the Annals of Pharmacotherapy. She serves on various system committees, including Executive Pharmacy and Therapeutics, Medication Safety, Electronic Health Record and Clinical Decision Support.
Tiffany Williams, Pharm.D., BCOP, CPh completed her Doctor of Pharmacy degree from Florida Agricultural and Mechanical University in 2008. After graduation, she worked in long term care and community pharmacy before becoming a Clinical Staff Pharmacist at Baptist Hospital of Miami in 2013. In 2014, Dr. Williams joined Sylvester Comprehensive Cancer Center, specializing in solid and hematological malignancies. She earned her Board Certification in Oncology Pharmacy in 2017, while working as an outpatient oncology Clinical Pharmacist for the Memorial Healthcare System. In 2018, Dr. Williams joined the Miami Cancer Institute as a Clinical Informatics Pharmacist, and then transitioned to the BHSF Pharmacy Informatics team, specializing in oncology. She serves on various oncology and pharmacy informatics committees.
