All services require a doctor’s prescription and appointments are recommended. Before scheduling an appointment, please have your prescription and insurance information available. If you do not have a prescription, please find a doctor and plan a visit to have a medical review. Our helpful staff will facilitate any necessary preauthorization with your insurance company before your visit.
